INFORMATION PURSUANT TO EU REGULATION 2016/679 (CDGDPR GENERAL DATA PROTECTION REGULATION)

Pursuant to article 13 of EU Reg. 2016/679 Orthopedics Sanitaria Cibrario provides the necessary information regarding the processing of personal data and communicates the following.

art. 1 – Data controller.

art. 2 – Purpose of the processing.

The data provided may be collected for one or more of the following purposes:

art. 3 - Processing and protection of data from access by unauthorized parties.

The data will be processed in the following ways:

Recording and processing on a) paper and b) digital media or c) via cloud storage.

The data processed in points a) and point b) are stored within the territory of the European Union.

The data referred to in points b) and c) are protected from access by unauthorized parties with specific security passwords, as well as through antivirus and firewall, encryption and multi-factor authentication.

In any case, access to the data referred to in this information by unauthorized parties is prohibited.

art. 4 – Duration of treatment.

The data collected with the consent of the interested party will be processed and stored for the period of time required to achieve the purposes for which the data were collected, and, subsequently, for the period of time necessary to fulfill the conservation obligations for fiscal or other purposes envisaged by laws or regulations, without prejudice to the right of the interested party to ask the data controller to delete the data or limit the processing that concerns him or her, or to oppose the processing, in addition to the right to portability referred to in the art. 8 of this information.

At the end of the retention period, the personal data will be deleted.

Therefore, upon expiry of this deadline the right of access, cancellation, rectification, limitation and portability can no longer be exercised.

art. 5 – Mandatory nature.

All the requested data are necessary to achieve the purposes referred to in the art. 2 of this information.

art. 6 – Type of data processed.

As required by the art. 9 chapter f) of EU Reg. 2016/679, the processing may consist in the storage and management of personal data provided consensually by the interested party and which, potentially, also contain religious or philosophical beliefs, data relating to health or sexual life, genetic data, biometric data intended to uniquely identify a natural person, as well as data containing banking, financial and tax information referable to you.

The above data, it is reiterated, are precluded from third parties, except in the cases referred to in the art. 7 of this information.

art. 7 – Subjects to whom the data may be communicated.

The personal data provided may be communicated to:

Subjects who must have access to the data to fulfill contractual and pre-contractual obligations established or to be established towards customers or pursuant to current legal provisions.

art. 8 – Rights of the interested party.

The interested party may revoke the consent previously given to the processing of his/her personal data at any time.

The data controller, upon request of the interested party, must provide without unjustified delay and, in any case, at the latest within one month of receiving the request,

the following services:

- access to personal data concerning the interested party and the right of the latter to receive a copy of the data processed;

- the limitation of processing, rectification, or partial or total cancellation of personal data (this may make it impossible to obtain the requested service or use the services offered by the site;

- data portability consisting in the delivery to the interested party of a commonly used medium readable by an automatic device of the data attributable to him, or, if technically feasible, the transmission of the data concerning him to another data controller identified by the interested party, against a cost contribution reasonably proportionate to the extent of the service requested.

d) the identification details of the owner, managers and designated representative in the territory of the Italian State, where applicable;

e) of the subjects or categories of subjects to whom the personal data may be communicated or who may become aware of them in their capacity as designated representatives in the territory of the Italian State, managers or appointees.

3. The interested party has the right to obtain:

a) the updating, rectification or, when interested, integration of the data;

b) the cancellation, transformation into anonymous form or blocking of data processed in violation of the law, including those whose retention is not necessary in relation to the purposes for which the data were collected or subsequently processed;

c) certification that the operations referred to in the previous letters a) and b) have been brought to the attention, also with regard to their content, of those to whom the data have been communicated or disseminated, except in the case in which such fulfillment proves impossible or involves the use of means that are manifestly disproportionate to the protected right.

4. The interested party has the right to object in whole or in part:
